Comparison of the Accuracy of Clinical Measurements and Cone-Beam Computed Tomography for Assessing Peri-Implant Buccal Bone Thickness Around Titanium and Zirconia Implants

Dental implants have been an effective solution for restoring oral function and aesthetics in patients with tooth loss. Currently, technological advances have introduced new materials, such as zirconia, which have sparked significant interest in the scientific community. This research aims to compare zirconia and titanium implants and highlight the importance of cone beam computed tomography (CBCT) in the diagnosis of bone complications such as buccal bone dehiscence or fenestrations. Dental implants are a growing part of the prosthetic field. Zirconia has gained popularity as an implant material due to its color and advantageous biological potential. Although it demonstrates soft tissue adhesion and osseointegration potential comparable to commonly used titanium, buccal bone thickness is typically assessed during implant installation or second-stage surgery in submerged implant protocols, but is rarely evaluated thereafter, as conventional oral radiographic diagnosis only assesses interdental marginal bone levels. This factor, however, can have important implications for the long-term stability of peri-implant hard and soft tissues. Computed tomography (CBCT) has significantly expanded the use of CT imaging in implant dentistry due to its potentially reduced radiation dose and cost. However, CBCT images of peri-implant bone can be affected by artifacts that can generate incorrect bone gray values and, consequently, erroneous reconstructions. This study aims to evaluate the accuracy of measuring peri-implant buccal bone using cone-beam computed tomography devices on zirconium implants compared to titanium implants. Objective: To evaluate the accuracy of cone-beam computed tomography on zirconium and titanium implants for measuring buccal peri-implant bone. Implants were placed at different distances from the cortical bone in one prepared porcine rib. The rib will be scanned in a reproducible position using a CT scanner. An examiner will take four measurements on the eight implants: cortical bone thickness from the outer surface of the implant threads at 0, 3, 6, and 9 mm from the top of the implant.
