El diagnóstico de la hidatidosis hepática se basa en la clínica del paciente, en las técnicas de imagen y en los estudios serológicos. Suele ser asintomática durante varios años y su sintomatología es muy variable dependiendo de la ubicación y tamaño del quiste; y nos da el diagnóstico presuntivo. La ecografía detecta aproximadamente el 90-96% de los quistes hidatídicos hepáticos, es eficaz, de bajo costo, útil sobre todo en regiones endémicas, sin embargo es menos precisa que la TAC para detectar y delinear la extensión del quiste, la sensibilidad de la TAC es del 97 al100%, y es útil cuando el diagnostico de hidatidosis es incierto o cuando se sospechan complicaciones como ruptura o infección de un quiste. Aunque el diagnóstico por imágenes (ecografía, TAC y RNM) tiene mayor sensibilidad global y aporta más datos, el inmunológico es de utilidad en los casos donde se trate de descartar patologías tumorales y o lesiones quísticas que pudiera ocasionar el parásito. Se usan combinaciones de 2 o más test, uno sensible como ELISA o Hemoaglutinación Indirecta, con un Arco 5 de Capron como confirmación. La especificidad de los métodos descriptos varía del 95 al 100%. En la serología hay varias pruebas que son específicas de la hidatidosis, pero la ausencia de serología positiva para el Echinococcus no descarta la enfermedad
The diagnosis of the hepatic hydatidosis is based on the clinic of the patient, the techniques of image and the serological studies. Usually is asintomatic during several years and its sintomatologic is very variable depending on the location and size of the cyst; and it gives the diagnostic presumtive. The ultrasonography approximately detects between 90 and 96% of the hydatid liver cysts, is ef fective, is not extensive, useful mainly in endemic regions, nevertheless is less precise than the com puted tomografhy to detect and to delineate the extension of the cyst, the sensitivity of the computed tomografhy is between 97 and 100%, and is useful when I diagnose of hydatidosis is uncertain or when to complications like rupture or infection are suspected a cyst. Although the diagnosis by images (ultrasonography, computed tomografhy and RNM) has greater global sensitivity and contributes more data, the immunological one is of utility in the cases where it is to discard tumors pathologies and/or cystic injuries that could cause the parasite. Combinations of two or more test, one sensible like ELISA test or Indirect Hemagglutination are used, with an Arc 5 of Capron confirmation. The specificity of the descripted methods changes from the 95 to the 100%. The serologic are several tests that are specific of the hydatidosis, but the absence of positive serolo gic for the Echinococcus does not discard the disease. - Fuente
