Laringotracheal complications associated with the time of permanence of artificial air pathway

Las complicaciones asociadas a la vía aérea artificial oscilan alrededor del 10 al 60%, éstas están relacionadas con el tiempo de permanencia, el tipo de vía aérea artificial y a otros factores como ser Diabetes Mellitus, Quemaduras, Enfermedades inmunológicas, Desnutrición, etc. Las complicaciones pueden ser tan sutiles como disfonía o comprometer la vida como las estenosis o las fístulas traqueales. El objetivo del trabajo fue describir la incidencia de las complicaciones asociadas a la vía aérea artificial y relacionar el tiempo con el tipo y momento de aparición de la lesión. Se incluyeron pacientes ingresados al Servicio de Terapia Intensiva, desde Julio de 2000 hasta julio de 2005, todos ellos debieron permanecer con vía aérea artificial por un tiempo mayor de 24 hs., se excluyeron aquellos que al ingreso presentaban vía aérea artificial o antecedentes de compromiso laringo-traqueal. Se analizó la incidencia de las complicaciones de aparición inmediata o tardía, el tipo de lesión, tiempo de permanencia de la vía aérea artificial, método complementario utilizado para el diagnóstico, tratamiento empleado o si debió ser derivado. Se incluyeron 47 pacientes, la edad promedio fue de 39 ± 16 años y los días de internación de 13 ± 7 días. Se observaron complicaciones inmediatas en 31 pacientes y tardías en 16 pacientes, el método complementario mas utilizado fue la fibroscopía; aquellos que permanecieron poco tiempo con vía aérea artificial desarrollaron complicaciones de fácil tratamiento y aquellos con vía aérea artificial prolongada requirieron un tratamiento más complejo y especializado.
The complications associated with the artificial air parthway oscilate between 10 and 60 % ap proximately. They are related to the time of permanence, to the artificial air parhway type and other factors such as Diabetes Mellitus, burn, immunologic illnesses, malnutrition, etc. The complications can be either subtile, like dysphonia, or dangerous, like stenosis or the tracheal fistulas. The aim of the work was to describe the incidence of the complications associated with the artifi cial air pathway as well as to relate the time with the type and moment of the appearance of the le sion. Those patients that ingressed to the Intensive Care Service were included, from July, 2000 to July, 2005, all of them had to remain with the artificial air pathway during a period of time longer than 24 hs. Those patients that had artificial air pathway or laringotracheal antecedents of compromise, when they ingressed, were excluded. The incidence of the complications of immediate or tardy appearance, the type of lesion, the time of permanence of the artificial air pathway, the complementary method used for the diagnostic, the employed treatment and possible shunt have been analyzed. 47 patients have been included, the averaging age was 39 ± 16 year-old and the days of hospitali zation were 13 ± 7 days. Inmediate complications in 31 patients and tardy complications in 16 patients have been observed. The complementary method that has been mostly used was the fibroscopy; those who remained a short period of time with artificial air pathway developed complications of easy treatment while those with a protracted artificial air pathway needed a more complex and specializer treatment.
